Infant Feeding, Diaper Change, Medicine, and Sleep Log (Excel 2010 Spreadsheet)

I didn't see any logs that I really liked so I made a basic one using Excel 2010. I included a check box for vitamin D since the American Academy of Pediatrics recommends 400 IU daily for breastfed infants. I also included some generic check boxes in case there are medicines that your baby needs to take.
Even though I have an iPad and Android device, it's just easier to have a paper log. At 3:00 AM in the morning you don't want to be fussing around with trying to enter data on a mobile device. Some things are just better doing them the old way.
BTW, for vitamin D, do not get the Enfamil D-Vi-Sol brand. It has a lot of unnecessary ingredients, including artificial color and flavor. I switched to the Carlson brand (see link below) which has only these two ingredients: vitamin D and fractionated coconut oil.
The latest version of this log is 1.6. Feel free to use and distribute, but give me credit. Print on both sides of standard 8 1/2" x 11" paper and punch three holes at the top so you can put them in a binder. For some reason Excel wants to print a second, blank page, so I explicitly set the print area in the worksheet so that it'll only print on one page.
Here's an example that I partially filled out:

Can you upload an example version that is filled out? I want to see how you envision it is filled out.
